Safe Transition

We provide early, clear and ongoing communication with the participant, health care staff and support networks, to ensure critical medical information is not lost during transition.

How we support you

• we prepare you to go to hospital, both for planned and emergency visits

• we keep participant’s health and medication information accurate and up-to-date

• we ensure you bring essential items for hospital admission.

In Emergency

We support the participant by having our support staff stay with them during the admission, and as needed while in hospital.

For planned hospital visits, we communicate with hospital staff beforehand

Discharge and Ongoing Care

• we plan for the participant’s discharge in consultation with health professionals as early as possible

• we ensure any new support needs are understood and in the plan before discharge

• we offer transition care accommodation on discharge from the hospital
